Yes, a patient or an individual can directly seek assistance from Liminal Health. We will need documentation of recent hospitalization in order to make a appropriate assessment of patient or individual condition including their needs. 

If a family member wishes to refer a patient or individual for assistance, it is recommended that they  talk to the hospital or facility social worker or discharge planner. The social worker or discharge planner will assess and identify patient’s discharge planning needs and can make the referral to Liminal Health. Both Liminal Health case manager and hospital /facility social worker and discharge planner will work together in determining resources to address patient’s discharge needs.

A simple phone call to Liminal Health will initiate the referral process. Another option is submitting an online application available at our website.
